• Ask Your Doctor: Request your physician to fax a referral to our office. This is the first step in setting up your personalized care plan.

• Coming Home from a Hospital or Nursing Home: If you're preparing to transition home and need skilled services, speak with your discharge planner or nurse. They can help arrange home health services with Americare.

Please note: You must have a Primary Care Physician of your choice to oversee your care and coordinate your treatment plan.
